Online consultations:
By booking and paying for an online consultation you understand the following:
1. You confirm and consent to participation in a video consultation;
2. You agree to the private charges as specified by Hexagon Health;
3. You understand that your health information is obtained to provide accurate and safe nutrition plans and services;
4. Your information will be kept confidential in accordance with the Privacy Act 1993;
5. You understand and may withhold or withdraw consent to the online consultation at any time without affecting any future consultations, by notifying us via email.
Cancellation & reschedule
We understand that unforeseen circumstances may appear prior to the consultations. By booking a consultation for any service we offer, you understand and agree to adhere with the minimum prescribed period and the cancellation notice period. You understand and agree to pay for the minimum prescribed period on a weekly basis. For any unforeseen circumstances, a payment has not been made, you agree to pay for the missed weekly payment the subsequent week with the current week payment. If you wish to reschedule an appointment, you agree and understand that we will choose the next available time and date, without affecting the fee paid for our service.
